Request for Repair Assistance

Please make sure to submit the form with all requested information to ensure timely review and follow-up.

Before getting started, please note the following:

• Once application is submitted, please allow 24-48 hours to hear from our team.

• Please allow 3 – 4 weeks for review and approvals, once application is complete. This is NOT an emergency repair service.

• Vehicle Title and bank statements must be in the applicants name or your repair application WILL NOT be accepted.

• We provide up to $500 in parts for approved repairs. All labor provided is voluntary in nature and does not carry a warranty.

• All recipients are ineligible to request additional repairs for 6 months after repair has been completed.

We are NOT designed to do the following:

• Provide long term repair services for your vehicle.

• Provide repairs considered cosmetic in nature.

• Provide loaner vehicles.

• Provide reimbursement for repairs or services completed outside of Drive Grace.
